- Download the Record Suspension Application Forms from the Parole Board of Canada website.
- You will need two pieces of valid government issued identification, one of which must have your:
- Full name
- Date of birth
- Photo
- Signature
- Third party consent (only if lawyer or private company is helping you with application)
Record Suspension Check
To submit a record suspension (pardon) application, you will need a record check done by the police agency where you live now and from every place you have lived in the past five years. This check must be done by the local police in each area.

Mail your Record Suspension Application forms to: London Police Service Please include a clear photocopy of your two pieces of identification. The fee can be paid by certified cheque or money order payable to the London Police Service.
If you would prefer to drop your application off, there is a drop box located just to the right of the Records Screening office inside Headquarters at 601 Dundas Street.

The cost of this check is $100. Fees should be made payable to "London Police Service" by way of:

We do not accept cash or provide refunds after commencement of process.
It takes seven days from the date we receive your request and this may change based on current demand.

You do not need a lawyer or private company to apply for a record suspension. If you choose to use a lawyer or private company to help you with your application, you must fill out a third party consent form. This allows the third party company to submit the form to us on your behalf.
