Name Change Application

• You must live within the City of London at the time you submit your form.
• Two pieces of valid government issued identification, one of which must have your:
  • Full name
  • Date of birth
  • Photo
  • Signature

If your current London address is not on your identification you must provide proof of your London address. Examples of this would include a utility bill or rental agreement.

Please be sure to provide the name that you will be applying to change to under the “Other first names or other last names section on the application form”. 

You must apply for your record check online (see the “Level 2 – Criminal Record and Judicial Matters Check” section for more information). If you are unable to apply online please call 519-661-5515 ext. 4788 for more information and assistance.

The cost of the record check is $74.00. The fee is payable by:

• Visa
• MasterCard
• American Express

We do not accept cash or provide refunds after commencement of process.

Record checks will be uploaded to your online account. You will receive an e-mail notifying you that your record check is complete and is available to download an electronic copy. Instructions will be provided in the e-mail.
