CONSENT FORM FOR POLICE RECORD CHECK
If you are applying for custody of a non-biological child and you have an upcoming court date, you can apply for this type of record check to accompany your form 35.1 application package. Please note that you CANNOT apply for this type of record check online. See the information below for more details.
Submit your form by mail
You will need photocopies of two pieces of valid government issued identification like a driver's licence or a passport, one of which must have your:
- Full name
- Date of birth
- Photo
- Signature
You must print a “Consent Form for Police Record Check for Non-Parent Custody Applicants” and submit it by mail to London Police Headquarters, 601 Dundas Street, London, Ontario, N6B1X1, attention Records Screening.

Fee and Payment
The cost of Non-Parent Applicants for Decision-Making Responsibility record check is $85. Fees should be made payable to "London Police Service" by way of:
Certified cheque or money order
We do not accept cash or provide refunds after commencement of process.
Processing Time
It takes approximately 1-2 weeks to complete this record check plus 5-7 business days for mailing.
Receiving your completed record check
Your check can only be delivered to you by mail. We do not offer in person pickup.
Important information
Please confirm with the court house or your legal representative that this is the correct level of record check required and that you are in fact the non-biological parent applying for custody. If you have access to the child but are not applying for custody as a non-biological parent you require the CRJMC (Criminal Records and Judicial Matters Check) only. It is also important to note that the court house will only accept this form within 60 days of having started your Non-Parent Applicants for Decision-Making Responsibility process.
